Insurance - Request Certificate of Insurance

A certificate of insurance (COI) is a document from an insurer to show you have liability insurance for an event. They're also known as certificates of liability insurance or proof of insurance.

It’s common to need a COI when planning an event. Venues often require hosts to have insurance and may contractually require the host to name the venue as an Additional Insured. A COI provided to the venue contains the information they need to confirm that you are properly insured.

Under CUPA-HR’s insurance plan, you have the option to request proof of liability for specific chapter events. To request a certificate of insurance, please complete the Request for Certificate of Insurance form. Please email your request at least one week prior to your event.
